2026 Awards for Excellence Nomination Form

Each year, exceptional performances in environmental, health, safety and security by companies in Ohio’s chemical industry are recognized by the Ohio Chemistry Technology Council (OCTC) through our Awards for Excellence. Nominations for awards are now open and will remain open until February 6, 2026.

Awards for Excellence Presentation

OCTC is excited to announce that the 2026 Awards for Excellence will be presented in-person at our awards dinner on Monday, May 18th, 2026 in conjunction with our 37th Annual Conference. The dinner will be attended by government, industry, and community leaders. Statewide publicity will be given to award recipients. Award recipients will be encouraged to invite community leaders, community advisory panel members, and employees involved in the award-winning activities.

Activities and Programs Eligible for Nomination

Each facility of a member company may submit a nomination on its own behalf. In addition to demonstrating commitment and achievement in protecting people and the environment, a nomination must focus on a program or activity the company believes represents excellence in environmental, health, safety or security performance. All nominations should be for activities in the 2025 calendar year. 

Eagle Award for Excellence

In addition, OCTC will present the Eagle Award for Excellence in Environmental, Health, Safety & Security Performance. This award will be presented to the award recipient whose performance is judged exemplary among the group of distinguished Excellence Award recipients. Our panel of judges will identify one award recipient that stands out for its scope, implementation, impact and sustainability.

SECTION I: Demonstrating Commitment to Excellence
Please note, a "no" answer in SECTION I will automatically result in disqualification of the nomination.

Has the senior manager of the entrant facility/company made protection of people and the environment a personal goal?

Has that goal been communicated to all employees?

Is implementation of programs to protect people and the environment captured as a key component in the facility's operating plans?

Do adequate management systems exist to support full implementation of programs to protect people and the environment, and to require a focus on continuous improvement in the areas of environmental management and protection of people?

SECTION II: Facility Performance in 2025
Please note, a "no" answer in SECTION II will not automatically result in disqualification of the nomination. However, the nominee will be asked to explain any incident in the context of the company's overall commitment to excellence in its programs to protect people and the environment.

Has the facility achieved zero fatalities?

Has the facility achieved good regulatory compliance standing resulting in no major enforcement actions, settlement activity, fines or penalties?

Has the facility had any fires, explosions or off-site releases resulting in public notification or impact?

Has the facility pursued pollution prevention through an on-going program to reduce wastes and releases, giving preference first to source reduction?

SECTION III: Community Advisory Panel

During 2025, did this facility have an active Community Advisory Panel or equivalent formal process providing meaningful opportunity for achieving community awareness, exchanging information with the community and receiving feedback from the community?

SECTION IV: Program Description

Please indicate the area of performance below from the following options:

Environmental Performance - emissions reductions, waste reductions or other reduction or sustainability achievements

Health and Safety Performance - worker safety, customer safety, transportation safety or other health or safety achievements

Security Performance - making facility operations more secure, working with government officials in joint security efforts or other security achievements

Please include below, or email separately, a description of the program or activity you believe exemplifies sustained commitment, excellence and/or innovative approaches to improving performance. Please keep in mind this version will be used by our moderators when presenting at the awards ceremony and will be printed in our conference materials as your program description.
